Paradoxical Adipose Hyperplasia

Paradoxical Adipose Hyperplasia (PAH) is a rare condition that can occur after certain fat-freezing treatments, most commonly cryolipolysis (commonly known by brand treatments such as CoolSculpting). Instead of reducing fat in the treated area, the fat tissue enlarges and becomes firmer over time.

In PAH, the affected area develops a noticeable, well-defined mass that is often larger than before treatment. The tissue typically feels rubbery or dense and maintains the same shape as the applicator used during the fat-freezing procedure. This reaction usually appears several weeks to months after treatment.

PAH is a benign (non-cancerous) condition, but it can cause cosmetic concern and emotional distress due to the unexpected increase in volume in the treated area. It does not typically resolve on its own and may remain stable or slowly progress over time.

Diagnosis is usually made based on clinical history and physical examination, particularly in patients who have undergone prior cryolipolysis. Imaging studies such as ultrasound or MRI may be used to confirm the nature of the tissue and rule out other conditions.

Treatment most often involves surgical correction, such as liposuction or excision, to remove the abnormal fat tissue. Non-surgical options are generally ineffective. In many cases, surgical management can significantly improve contour and appearance.
