Important Information About Estimates

Estimates are not guarantees of your final bill, and your actual charges may change based on your diagnosis, treatment needs, and the services ordered by your physician. They generally do not include separate professional fees (such as your physician, anesthesiologist, radiologist, or pathologist), and your insurance benefits, remaining deductible, copay, and coinsurance will affect what you owe. Final coverage decisions are made by your health plan when your claim is processed, and you are responsible for any services your plan does not cover. If your bill is at least $400 more than your Good Faith Estimate, you may be able to dispute the bill through the federal patient‑provider dispute resolution process.
